Detox needs and assessment
Is a detox assessment needed, and who makes that clinical decision? What withdrawal care can this provider safely deliver, and what would require another setting?
A practical guide for your next conversation
Use the same questions with two treatment providers. Record what each provider confirms, where the information came from and what still needs checking.
This is a conversation aid, not a clinical assessment, provider rating or recommendation. A treatment provider must assess whether it can meet the person’s needs. An unanswered safety question needs clarification, not a score.
Use provider names and service information only. Do not enter anyone’s name, health history or other personal details. Notes stay in this open page and are not sent to IRN or saved by the worksheet. Save or print before closing or reloading; keep any saved copy private.
Establish the required level of care before comparing location, accommodation or price.

How will the provider consider previous treatment, what helped or did not help, and the person’s likely response to its approach? Keep personal history out of this worksheet.
Who provides medical, mental-health and therapeutic support? What are their qualifications, availability and supervision, and what does a typical week include?
Which regulator covers this service and location? Record the current registration, inspection report and complaints route, where applicable.
Compare dated, written quotations for the same proposed duration and assessed needs.
What is the total price, currency and duration? How are the duration and any extensions decided, reviewed and charged?
Are assessment, medication, medical appointments, therapy and aftercare included? Ask for every expected separate charge in writing.
What are the travel, transfer and other practical costs? What are the deposit, cancellation, refund and early-discharge terms?
Who is being paid by whom? Does any adviser receive a referral payment, does it affect the price, and how were these options selected?
Discuss continuing support before admission, including who will arrange it and what it will cost.
Who prepares the discharge plan? Which follow-up appointments or services will be arranged, and who takes responsibility for the handover?
What local, online or community support can continue at home? What is included, for how long, and what would be paid separately?
What support can relatives receive in their own right? How will consent, confidentiality and family involvement be agreed?
Which points remain unconfirmed? Who will clarify them and by when? Record the next conversation needed before making a decision.
